Referral Required: We kindly ask that patients be referred by a primary care physician or specialist to ensure coordinated care and a thorough evaluation of their neurological needs.

Insurance: We accept Medicare, PPOs, and select HMOs. Please verify your coverage directly with your insurer to confirm in-network status, as we are not responsible for out-of-network limitations.

What to Bring: For your first visit, please remember to bring your referral, insurance card, and photo ID.

Medical Records: If available, please bring any recent test results (MRI, CT, EEG, EMG, lab work) and prior neurology or hospital records.

Medication List: Bring an updated list of your current medications, including dosages and over-the-counter supplements.

Arrival Time: Please arrive 15 minutes early to allow time for check-in and paperwork.

Cancellation Policy: We kindly ask for at least 24 hours’ notice if you need to cancel or reschedule. Appointments not canceled within this time will incur a $50 fee.
